Camera Registration Program

In an effort to continue our public safety partnership with citizens and businesses, the Melrose Park Police Department is offering a security camera registration program. The Melrose Park Camera Registry Program (hereinafter the “Program”) allows residents and business owners to voluntarily register the locations of their video surveillance systems with the Melrose Park Police Department. Such registry will allow the Police Department to identify the locations of nearby video cameras to obtain potential video surveillance, whenever a crime occurs. The Police Department may then ask Program participants to review their private video surveillance systems for a specific date and time for video that may show activity involved with a crime. If they so choose, participants in the Program can then voluntarily provide copies of such video footage to the Police Department, for investigative purposes.

DISCLAIMER AND TERMS OF USE 

By signing below, you acknowledge that you have read and understand the following Melrose Park Camera Registry Program terms and conditions and agree to such terms and conditions as follows:

Your participation is voluntary. The private information that you supply will be kept confidential, unless subject to disclosure by court order or as otherwise required by law. Only law enforcement personnel can view the surveillance video.

Your participation in the Program does not provide the Melrose Park Police Department with direct access to your camera and/or surveillance equipment.

Your participation in the Program does not make you an agent or an employee of the Melrose Park Police Department or the Village of Melrose Park.

The Melrose Park Police Department and the Village of Melrose Park do not and shall not determine the location for the installation or the use of your video equipment.

You may withdraw from the Program and delete your registration at any time by notifying the Melrose Park Police Department, 1 N. Broadway Ave. Melrose Park, IL 60160: Attn: Deputy Chief of Police.

Upon your withdrawal from the Program, your name shall be removed from the list of the locations of registered video surveillance equipment. If you withdraw from the registration program, you must reimburse the village any amount you received toward the purchase of the camera equipment.

If video footage from equipment that you register may assist in the investigation of a crime, the Melrose Park Police Department will contact you, using the contact information that you provide, in order to request that you provide the video footage to the Police Department. Such video footage may be used as evidence during any stage of an investigation or criminal proceeding.

Video footage provided by you to the Melrose Park Police Department, to the extent permitted by law, will remain confidential.

Neither the Village of Melrose Park, the Melrose Park Police Department, nor their officers or employees make any assurances to you of any kind due to your participation in the Program.

You are solely responsible for using and maintaining your equipment in compliance with all applicable laws. Your participation in the Program is not a determination by the Village of Melrose Park Police Department or the Village of Melrose Park that you are using and maintaining your surveillance equipment in compliance with all applicable laws.

The camera program is a 50/50 reimbursement up to $100.00 for new purchases only, (example: cost of camera is $220 then the Village will reimburse $100, cost of camera is $150 then the Village will reimburse $75).

The registration, proof of purchase, and inspection of the equipment must be completed before funds are issued.

You will be contacted and required to email a receipt and screenshot of the view of the camera after submitting this form.
